Move 'Submit' button into header
If a change is ready to submit the natural next step is to submit it,
hence the 'Submit' button should be displayed in a more prominent
place. This is consistent with displaying other buttons in the header.
The idea of having the 'Submit' button in the header was already
discussed before [1]. There was the concern that if the 'Submit'
button appears right at the place of the quick approve button, then
it's too easy to accidentally submit the change after quick approve
has been done. This is a disadvantage of moving the button to the
header.
